RESOLVED 13/7/21 11:00. NHS Digital have made changes to resolve the impact to practices

A number of England practices are reporting issues connecting to services, including Vision and Consultation Manager, using smartcards. If you are affected by this please work offline (without smartcards) as a workaround. We are investigating possible causes at this time
